TRAVEL WAIVER & RELEASE DETAILS
This Waiver and Release of Liability is executed by and between illFX Entertainment Inc, herein referred to as the "Studio," and the undersigned parent or legal guardian (herein referred to as the "Parent/Guardian") of the minor child (“Minor”) named above, for the purpose of attending and participating in the Studio’s travel events and related activities.
ACKNOWLEDGMENT & ASSUMPTION OF RISK
The Parent/Guardian, acknowledges and understands that participation in the Studio's travel events involves inherent risks, including but not limited to physical injury, accidents, property damage, loss, or other unforeseen events. The Parent/Guardian agrees that the Minor's participation is voluntary and understands these risks.
And the Parent/Guardian hereby assumes all risks, whether foreseen or unforeseen, associated with the Minor’s participation in the Studio's travel events, including but not limited to travel, physical activity, and accommodations.
MEDICAL RELEASE
The Parent/Guardian grants permission for Studio staff to seek medical attention for the Minor in case of an emergency or if deemed necessary by an attending professional. The Parent/Guardian agrees to be responsible for any and all medical expenses incurred.
RELEASE OF LIABILITY
In consideration of the Minor's participation in the Studio's travel events, the Parent/Guardian, on behalf of themselves and the Minor, agrees to release, discharge, and hold harmless the Studio, its owners, staff, agents, and affiliates, from any and all claims, liabilities, causes of action, injuries, damages, or losses, including but not limited to personal injury or property damage, that may arise from the Minor’s participation in travel events, even if caused by the negligence or other fault of the Studio.
INDEMNIFICATION
The Parent/Guardian agrees to indemnify, defend, and hold harmless the Studio, its owners, staff, agents, and affiliates from any and all claims, actions, damages, or losses arising out of the Minor’s participation in the event, including those arising from the Parent/Guardian’s breach of this Agreement.
SUPERVISION
The Studio will provide reasonable supervision for the Minor during the events. However, the Parent/Guardian acknowledges and agrees that the Studio is not responsible for the actions or conduct of the Minor outside of the scheduled event activities.
CONSENT TO PHOTOGRAPH & RECORD
The Parent/Guardian grants permission for the Studio to take photographs and/or video recordings of the Minor during the travel event for promotional, advertising, or educational purposes, without compensation or further consent required.
SEVERABILITY
If any part of this Agreement is deemed invalid or unenforceable, the remainder of the Agreement shall remain in full force and effect.
